  • The Division of Insurance

  • The Secretary of Health and Human Services

Complaints to our office should be submitted in writing and should include as much detail as possible. You will not be retaliated against for filing a complaint.


6. Confidentiality and Security

We restrict access to non-public personal financial and health information to employees who need access in order to provide products or services to you.

We maintain physical, electronic, and procedural safeguards that comply with federal regulations to protect your non-public personal financial and health information.


7. Dispute Resolution

Any controversy or claim arising out of or relating to this Privacy Policy, or the breach thereof, shall be settled by binding arbitration in accordance with the rules of the American Arbitration Association. Judgment upon the award rendered by the arbitrator(s) may be entered in any court having jurisdiction.
